Se as pessoas em estados vegetativos persistentes não têm objetivos, elas não são estritamente agentes morais. Para ser um agente moral , elas teriam que fazer escolhas e realizar ações intencionais; normalmente, ações pelas quais elas podem ser culpadas ou elogiadas. Elas não realizam tais ações. Como não são agentes morais, não têm status em relação à saúde como pessoas. Mas isso não significa que elas não tenham nenhum estado de saúde. Ainda podemos avaliá-las com relação à saúde como organismos.
Obviamente, tendemos a pensar que os objetivos de uma pessoa são relevantes para o que acontece com o corpo e a propriedade da pessoa, mesmo depois que ela deixa de ser, a rigor, portadora de objetivos.

If people in persistent vegetative states have no goals, they are not strictly speaking moral agents. To be a moral agent, they would have to make choices and perform intentional actions; typically, actions for which they might be blamed or praised. They perform no such actions. As they are not moral agents, they do not have a status with respect to health qua persons. But this does not mean that they have no health status whatever. We may still evaluate them with respect to health qua organisms.
Of course, we do tend to think that a person ’s goals are relevant to what happens to the person’s body and property even after the person ceases to be, strictly speaking, a bearer of goals.
